Recurrent Hypertrophic Pachymeningitis Successfully Treated with Cyclophosphamide

Abstract
Hypertrophic pachymeningitis is a inflammatory disorder that causes fibrous thickening of the dura mater, presenting with headache, diplopia, focal deficit, and seizure. High-dose corticosteroid is generally accepted as the first-line treatment. Some cases were reported to be refractory to corticosteroid, and immunosuppressive drugs lead to clinicoradiologic resolution in such cases. Herein, we reported a patient with relapsing idiopathic hypertrophic pachymeningitis in whom neurological symptoms and radiological abnormalities were refractory to corticosteroids and were successfully reversed with cyclophosphamide treatment.
